Specialty Assessment Terms & Agreements

You have been invited to be a research participant in a research project we are doing. We want to review for you a few things about your participation in thisresearch:

• Taking part in this research is voluntary. Whether you take part is up to you.
• You can choose not to take part. There will be no penalty or loss of benefits to which you are otherwise entitled.
• You can agree to take part and later change your mind. There will be no penalty or loss of benefits to which you are otherwise entitled.
• If you don't understand, ask questions.
• Ask all the questions you want.

Here is a review of the research process itself:

• The research will take about 4 - 6 months.
• Your part would be simply to take a STAGES inventory and that takes about an hour to do.
• You will take the inventory here, online, as soon as you click the agreement box below.
• The inventory asks you to complete 36 sentence starters, such as: "I am—" (and you finish the sentence).

You might wonder if there are any drawbacks to taking this inventory. For the most part, these sentence starters and your completions wouldn't make you any more uncomfortable than your regular life does, and if you find yourself uncomfortable you can decide to withdraw, or simply skip that sentence starter and go on through the rest of the test.

However while we can't promise benefits from this process, we have had many people report to us that taking the inventory helped them to reflect on themselves in beneficial ways.

There will be no pay for taking the inventory and as well, there will be no costs either.

We want to tell you what we will do with the information we collect for this research:

• Your name, age, gender, profession, will be collected but it will be stored in a separate part of our online platform and no one will be able to connect any of this private information to what you write.
• The researchers will work with all of the sentence completions that all of the test takers provide. They will write a report on what they find. However they will not quote your sentence completion but will use general categories that represent everyone in their report.
• We may publish our research, but your name and private information will be kept confidential. We protect your information from disclosures to the extent required by law. We cannot promise complete secrecy.
